What's included:

• A 60 - 90 min consultation/general assessment to discuss your background including milestones, physical health, difficulties with performing your Activities of Daily Living (ADL's), interests and goals for treatment.  Assessments can be performed remotely OR in your home if you require assessment for home adaptation/equipment - Only available within 3 hour driving distance of Glasgow.
• A comprehensive report which you can share with health/employment professionals, which includes recommendations for further treatment/equipment and or reasonable adjustments for working.
• A remote feedback session to discuss the report and recommendations.
• Letters which can be passed on to your employer, education provider or health representative for further support.
